Please print and fill out one or more of the following forms.  Completed forms and any required remittances (check or money orders only-no cash accepted) may be returned to this office Monday-Friday, 8:00-5:00, or mailed to:  Town of Waterville,    P. O. Box 580, Waterville, WA  98858-0580.

SERVICE ORDER-MOVE IN:  For customers moving to a new residence in Waterville.  Please indicate a Scheduled Date and complete the CUSTOMER INFORMATION section listing a Waterville P. O. Box and street address.  Please include a good credit letter or remit a $100.00 deposit, signing where indicated to initiate service.

SERVICE ORDER-MOVE OUT:  For customers moving out of a residence in Waterville.  Please complete the CUSTOMER INFORMATION section listing a Waterville P. O. Box and street address for the current and new address.  Customers moving to another residence within the town of Waterville will need to complete both a SERVICE ORDER-MOVE OUT and CUSTOMER SERVICE-MOVE IN form to avoid duplicate billing.

CUSTOMER SERVICE REQUEST/SERVICE ORDER:  For customers requiring new, temporary, upgraded, relocated, or removed electric service.  Please fill out the CUSTOMER INFORMATION and SERVICE INFORMATION sections, check all applicable boxes, and sign and print name where indicated.  Please fill out the shaded areas of the accompanying SERVICE ORDER page.
